## Summary

不妊・不育症の治療を受けるご夫婦の経済的負担を軽減するため、治療費の一部を助成します。一般不妊治療および不育治療の自己負担額の2分の1（年度につき5万円まで）を助成し、最長5年間の助成期間を設けています。

## Eligibility
- 夫または妻が一般不妊治療等を受けた日以前から申請日まで、本町に住民票がある（事実婚を含む）
- 夫婦のいずれもが医療保険各法の被保険者、組合員、被扶養者である
- 夫婦のいずれもが町に納めるべき税等に全てに滞納がない
- 治療開始時における妻の年齢が43歳未満
- 夫婦の住所が異なる場合、他の市町村で助成を受けていない
