## Summary

- 父母が離婚した児童を養育している方
- 父または母が死亡した児童を養育している方
- 父または母が重度の障がい（身体障害者手帳1級から2級程度）を有する児童を養育している方
- 父または母が生死不明である児童を養育している方
- 父または母に1年以上遺棄されている児童を養育している方
- 父または母が裁判所からのDV防止法保護命令を受けた児童を養育している方
- 父または母が法令により1年以上拘禁されている児童を養育している方
- 婚姻によらないで生まれた児童を養育している方
- 父母が不明な場合（棄児等）の児童を養育している方
- 請求者が町に住民登録をしていること
- 児童が国内に住民登録をしていること
