## Summary

不育症治療に要した費用の一部を助成します。2回以上の流産や死産、早期新生児死亡の既往がある方で、夫婦いずれかが上川町に住所を有し、町税等に滞納がない方が対象です。1回の治療期間につき20万円を限度として助成します。

## Eligibility
- 2回以上の流産や死産、あるいは早期新生児死亡の既往がある方
- 不育症治療の開始日及び申請日において、夫婦いずれかが上川町に住所を有する者
- 申請日において、夫婦とも町税等に滞納者がいない者
- 被保険者等又は生活保護法に規定する医療扶助の対象外の治療を受けた者
- 他の市町村から不育症治療の助成を受けていない者
